The CIS Department of Clarion University of Pennsylvania requests the help of alumni from the CS or IS programs who graduated either 2 years ago or 5 years ago.

Alumni can complete the survey two different ways:
Option 1 : Download, Print, Fill in and Mail
  1. Print Alumni Survey (pdf file)
  2. Fill out the form using a pencil or pen
  3. Mail the form to:

    Clarion, PA 16214
Option 2: Download, Fill in and E-mail
  1. Download Alumni Survey (doc file)
  2. Complete the form in Word (or compatible software)
  3. E-mail the form as an attachment to: nHarriger@clarion.edu
